Job Summary

Provide quality, clinical services to patients in various community settings. Deliver patient care aligned with organizational philosophy, policies, and standards of community health practice. Manage admissions and case responsibilities according to MGB Healthcare at Home policies, utilizing electronic documentation and interdisciplinary communication. Offer skilled nursing assessments, teaching, treatment, documentation, and care coordination with high-quality customer service and financial awareness.

Qualifications and Experience

• Graduate of an approved School of Nursing
• Current Massachusetts license as Registered Nurse
• Minimum of one-year nursing experience in an acute care setting
• Prior home care experience preferred
• Travel within MGB Healthcare at Home area required
• Proficiency with computer software; knowledge of patient care devices preferred
• Strong interpersonal, customer service skills, and ability to work with diverse populations
• Effective communication skills in writing, verbally, and electronically
• Ability to work within an interdisciplinary team
• Organizational and prioritization skills, adaptability to changing situations
• Ability to work independently, be self-directed, and handle unpredictable circumstances
