Job summary

We are focused on improving medical patient safety through the admission, transfer and discharge process. We are currently looking for a Band 5 Registered Nurse to join our team, where the successful candidate will advance their skills in nursing assessment, admission, treatment of various medical pathways, and the transfer and discharge process.

Main duties
  • Communicate effectively with patients and their relatives, ensuring accurate and seamless information is conveyed and documented.
  • Maintain accurate and legible patient records in line with professional and legal requirements.
  • Act as the identified nurse for critically ill patients by assessing, planning, implementing and evaluating their care.
  • Participate in the planning of effective, timely and safe discharges.
  • Eliminate avoidable infections by serving as a role model in infection prevention and control for all staff, visitors and patients.
  • Provide training and support to new and junior staff in accordance with Trust policies and procedures.
  • Identify and minimise clinical and non‑clinical risks to reduce harm to patients, staff and visitors.
  • Involve patients in their care and ensure their understanding of treatment plans and care options before obtaining informed consent.
  • Maintain the privacy and dignity of patients at all times, working to minimise mixing of male and female patients and sharing of bathroom/toilet facilities.
  • Participate in department meetings when required.
  • Ensure compliance with professional expectations as outlined in the NMC Code of Professional Conduct.
  • Participate in an annual appraisal and follow a personal development plan as agreed with the line manager.
