Job Summary
We are looking for two proactive and skilled Practice Support Pharmacists to join our PCN teams, providing expert medicines optimisation, conducting structured medication reviews, supporting safe and evidence‑based prescribing, and collaborating with GPs, community pharmacies, and other healthcare teams across Stockport to improve patient outcomes. Our service operates between 8am and 6.30pm with flexible shifts available, with occasional requirements to support GP extended access working up to 8pm and weekend work may be required on occasion.
Key Responsibilities
- Support Primary Care Networks in the delivery of the National Directed Enhanced Service framework.
- Provide specialist knowledge, advice, and guidance on prescribing and pharmaceutical matters to patients, practices and other prescribers.
- Respond to medication queries and support practices as needed.
- Take part in and assist others with clinical audit work on prescribing standards, activity and practice.
- Participate in teaching sessions to ensure that pharmaceutical care problems are identified and communicated.
- Assist in coordinating solutions to supply problems with medicines used within the specialty.
- Work across boundaries of tertiary/secondary/primary care, coordinating with GP surgeries and community pharmacies to ensure seamless discharges.
- Ensure good quality patient care and demonstrate clinical reasoning and judgement.
- Ensure consistent compliance with local antimicrobial and GMMMG policies for medicines management.
- Deliver cost‑effective prescribing advice within specified contracts and an overall quality and clinical effectiveness framework.
- Conduct clinical structured medication reviews.
- Support and influence prescribers, work with the wider prescribing/medicines management system to influence change.
- Where qualified, prescribe medicines for agreed patients within their capacity as an Independent Prescriber.

- Experience – 2 years post‑registration experience or equivalent primary care experience.
- Other job requirements – Flexibility in working hours; active independent prescriber; ability to travel within Stockport.
Qualifications & Skills
- Qualifications – Vocational master’s degree in pharmacy or equivalent; 1 year UK pre‑registration training; registration with the GPhC; evidence of high‑standard CPD.
- Knowledge and Skills – Manage common clinical conditions (e.g. diabetes, cardiovascular disease); strong written, verbal and presentation skills; advanced competence in NHS pharmacy, IT, data management, audit, problem‑solving, and change management; good interpersonal, influencing and negotiating abilities.
- Desirable – Experience of GP clinical systems such as EMIS; completion of the CPPE pathway or equivalent; independent prescriber qualification; post‑graduate diploma; evidence of venepuncture competency; incident reporting and management skills.

Location: Merseyway Innovation Centre, 21‑23 Merseyway, Stockport, SK11PN.
Contract: Permanent, full‑time (37.5 hours), Band 7, salary £47,810‑£54,710 per year (Agenda for Change).
Working pattern: Flexible, with shifts from 8 am to 6.30 pm (occasionally up to 8 pm) and occasional weekend work.
