Job description

To provide a high quality care service based on theneeds of the Practice population which includes treatment, screening,preventative care and patient education

Main duties of the job

Help the practice develop and deliver specialistprimary nurse led services, including chronic disease management (including forall areas of QOF). Manages programmes of care for patients with chronicdiseases by planning, providing and evaluating care under agreed guidelines andpatient group directions. Maintains disease registers in liaison withadministrative staff. Undertakes annual reviews of patients understanding andability to self-manage.

Job responsibilities

*The job role is bank work only - shifts are adhoc and not guaranteed

Participate in national screening work such as cervicalcytology and deliver the national immunisation and vaccination schedule.Maintaining robust call and recall systems.

Administer child and adult immunisations and vaccinationsin accordance with national and local programmes. Competent in anaphylaxisand resuscitation techniques. Ensuring safe storage, rotation and disposalsof vaccines and drugs and that adequate stock levels are maintained

Cytology/smears

Provide comprehensive travel health advice for patientsprior to travel including appropriate vaccination e.g. yellow fever, safesex, sun protection, food hygiene, first aid and emergency medication, healthinsurance and vaccinations

Make professionally autonomous decisions for which he/sheis accountable and provide safe, evidence based, cost effective,individualised patient care

Care for patients with uncomplicated wounds including butnot limited to removal of sutures/tissue viability assessment/wound cleaningand dressing/management of burns and scalds/ ulcer care including Doppler assessment.

Carry out screening/testing including, but not limited tourine testing, Blood pressure monitoring, glucose testing, weight and heightmeasurements.

Make appropriate referrals to other members of theprimary, community and secondary care teams

Break bad news to patients/parents/carers and provideappropriate ongoing support to meet identified needs

Enables, supports and encourage individuals, families andgroups to address issues which affect their health and social well-being.

Promote health and well- being, giving general educationadvice on diet, contraception, smoking chronic disease management, exerciseetc

Where necessary evaluate patient test results inconjunction with the patient and their GP and ensure the appropriate courseof treatment is initiated.

Liaise and maintain good working relationships with allmembers of the practice team. Work within the multi-disciplinary team withinthe practice and across the wider health system

Administration

To maintain accurate patient records according to NMCStandards

Ensure accurate completion of all necessary documentationassociated with patient health care

Ensure all data protection requirements are met whengathering, recording and storing patient data.

To collect data and participate in regular Practice auditand research projects

To contribute to the preparation of any Practicedevelopment plans

Teaching andMentoring Role

Promote a learning environment for patients, nurses andother professionals

Assist with the planning and implementation and teachingfor practice staff including medical students, nurse students, nurses andhealth care assistants

Participate in clinical supervision

Managerial Role

As part of the nursing team ensure all appropriate practicepolicies are fully implemented.

Maintenance of stock and supplies, including clinicalsupplies, vaccines and health information literature, and providing timelyand accurate purchasing records to the practice manager

Participate in maintaining CQC requirements includingInfection control audits, COSHH standards, Cold chain, handling sharps andclinical waste.

General

To maintain own annual registration with the NMC andadhere to Code of Professional Conduct

To maintain a safe andtherapeutic environment for all PHCT staff, patients, carers and co-workersfrom other agencies. Ensure all statutory regulations (e.g. Health &Safety, COSHH, etc.) are implemented and adhered to

To promote a professional approach at all times by attireand attitude to patients and staff both in and out of the Practice

To maintain own continuous professional development andensure PDP kept up-to-date in line with the requirements of the Practice

Undertake regular mandatory training.

Attend and participate in staff and Practice meetings andin-house clinical training sessions

Any other reasonable duties that may be required from time-to-time
