A local medical practice in Truro is seeking a dedicated Practice Nurse to join their team. The ideal candidate will have at least two years of post-registration experience managing long-term conditions such as diabetes, asthma, and cardiovascular disease. Responsibilities include providing care, supporting patients, and working in a collaborative environment with GPs and fellow nurses. This is a part-time role with 24 hours per week over three days.

Based in Truro city centre, serving over 18,000 patients, this is an exciting opportunity for an experienced practice nurse to contribute to this busy practice. This post will be ideal for someone who is passionate about supporting patients who are living with a long-term condition, and who has at least two years post qualification experience in managing long-term conditions.
We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisation and communication skills.
Practice Nurse experience is essential as is a minimum of two years post-registration experience of providing nursing care to patients with diabetes and other long-term conditions, including asthma, COPD, and cardiovascular disease.
The role requires resilience and the ability to be flexible and adaptable. The successful applicant will be working alongside GPs and experienced nurses to offer care, advice, support and education to patients with long term conditions. Working both independently and with our other Practice Nurses, you will deliver care to patients who have been diagnosed with diabetes and other long term conditions e.g. asthma, COPD, cardiovascular disease as well as undertaking those duties usually undertaken by a Practice Nurse.
The Three Spires Medical Practice operates from Truro Health Park and we have a dedicated team of GPs, nurses, phlebotomists and physiotherapists caring for over 18,000 patients. We are a learning practice, hosting nursing students, GP trainees, and medical students from the Peninsula School of Primary Care, the University of Exeter and Plymouth University.
The post holder will undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs, other Practice Nurses or independently as appropriate for the need, according to Practice policies and protocols. The post holder will strive to improve standards of care and be involved in identifying Practice population needs.
