Job description

We are currently seeking an expereinced and dedicated Practice Nurse to join ourhealthcare team. The successful candidate will play a vital role in deliveringhigh-quality patient care within a busy clinical environment between our Havant andBedhampton sites. This position offers an excellent opportunity for a motivatedindividual to utilise and develop their nursing skills and contribute positively to patientoutcomes. You will be responsible for providing comprehensivenursing services, supporting our doctors and health care assistants, and ensuring the smooth operation of thepractice.

Main duties of the job

Your daily duties will be providing care and interventions following a list of patients who are booked into appointment slots. Their attendance could be for a variety of reasons which could include smears, chronic disease monitoring, wound care and dressings, immunisations and diagnostic tests like dopplers and ECG. Its a role with variety that allows you to practice autonomously but also be able to access support and advice from our experienced team of professionals who are on site when required.

Blossom Health are a GP practice based over 2 sites in Havant and Bedhampton. We have a caseload of approximately 16,000 patients. Our patients can access GP, Nursing and HCA appointments alongside accessing practitioners like physiotherapists, pharmacists and social prescribers through our involvement being within a Primary Care Network (PCN). Job responsibilities

Clinical

  • Manage patientswith chronic conditions by running dedicated clinic sessions.
  • Participate innational screening programs, including cervical cytology.
  • Provide travelhealth advice and administer appropriate vaccinations.
  • Deliver thenational immunisation and vaccination schedule.
  • Offer familyplanning services, oral contraceptive and depo checks, general contraceptionadvice, and HRT guidance.
  • Promote healtheducation and lifestyle advice to patients.
  • Administerprescribed injections (e.g., Zoladex, Prostap, B12).
  • Perform woundmanagement, including doppler assessments, compression bandaging, and removalof clips or sutures.
  • Select, perform,and interpret specific diagnostic tests
  • Collaboratingwith multidisciplinary teams to ensure holistic patient care.
  • Ensuringinfection control standards are maintained.
  • Act as achaperone when required.

Administration

  • Maintain accuratepatient records using agreed Read Codes on the clinical system.
  • Ordervaccinations and treatment room supplies.
  • Support teachingand training for HCAs and other team members.
  • Ensure completionof all documentation and tasks related to patient care.
  • Collect data andcontribute to practice audits and research projects.

General

  • Maintain annualNMC registration alongside revalidation and adhere to the Code of ProfessionalConduct.
  • Ensure a safe andtherapeutic environment for staff, patients, carers, and visitingprofessionals. Comply with all statutory regulations (e.g., Health &Safety, COSHH).
  • Alwaysdemonstrate professionalism through appearance and attitude, both within andoutside the practice.
  • Maintaincontinuous professional development and keep your CPD up to date in line withpractice requirements.
  • Attend andactively participate in staff meetings, practice meetings, and in-houseclinical training sessions.
  • Excellent patientcare skills with a compassionate approach
  • Ability to workeffectively both independently and as part of a team
  • Undertake anyother reasonable duties as required.

Safeguarding

  • Usingprofessional curiosity to identify patients at risk that may requiresafeguarding intervention.
  • Notifying thesafeguarding lead of any potential concerns identified during their dutieswhich may place a person at risk of harm or abuse.

Experience

  • Strong knowledge of NHS protocols and primary care processes.
  • Excellent organisational and leadership skills.
  • Proficiency in clinical systems and Microsoft Office.
  • Ability to work under pressure and prioritise effectively

  • Registered General Nurse with current NMC Registration and no restrictions on practice.
