Main duties of the job

As our new Practice Nurse, you will play a vital role indelivering a wide range of nursing services, including chronic diseasemanagement, immunisations, cervical cytology, wound care, and health promotion.

Working as a member of the practice multidisciplinary team, the successfulcandidate will ensure nursing services are delivered effectively to theentitled patient population.

Our Nurse Team are responsible for a number of clinical areas suchas infectionprevention and control, health promotion, chronicdisease management, health promotion, well women and well man clinics, as wellas actively supporting the practice management team in the reviewing anddelivery of clinical policy and procedure.

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  • Dressings and wound care to include suture removal.
  • Administer IM injection and vaccinations under PGDs including travel vaccinations.
  • Cervical smear tests (training available).
  • Leg ulcer care including multi-layer and pressure dressings.
  • Monitor and manage chronic conditions, providing education and support to patients.
  • Collaborate with doctors and other healthcare professionals to develop patient care plans.
  • Maintain accurate patient records in compliance with confidentiality regulations.
  • Provide health education to patients and their families regarding disease prevention and health promotion.
  • Monitor stock levels and support with ordering.
  • Ensure the practice environment is clean, safe, and well-organised.
  • Keeping patient records up to date.
  • Checking a patients temperature, pulse and blood pressure correctly.
  • Working alongside and communicating regularly with practice GPs, nurses, pharmacists and other healthcare professionals.
  • Emergency equipment audits and checks.
  • To assist GPs with minor surgery when required
  • To implement vaccination programmes for adults and children
  • To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
  • To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • To support the clinical team with all safeguarding matters, in accordance with local and national policies
  • To deliver opportunistic health promotion where appropriate

Experience
  • - Experience of working in a GP Practice
  • - Chronic Disease Management would be beneficial but we are also happy to upskill
