What you will do:

  • Assess and provide tailored physiotherapy interventions to individuals within the reablement service.
  • Work collaboratively with an MDT (multi‑disciplinary team) to develop integrated care plans.
  • Support patients in community settings, assisting in their rehabilitation and reablement.
  • Document patient progress and offer insights for case discussions.
  • Provide guidance and advice to patients and their families, enhancing their rehabilitation journey.

Requirements of the Physiotherapist:

  • Must possess a valid UK driving licence as a car driver.
  • Registration with the Health and Care Professions Council (HCPC) is essential.
  • Experience working within a multi‑disciplinary team and in rehabilitation is preferable.
  • Seeking a candidate with some post‑graduate experience to manage complex team caseloads effectively.
  • Ability to work across all pathways of Intermediate Care, with some community experience required.
