This is an opportunity to work on a flexible basis, supporting our growing rehabilitative services. Youll be part of a multidisciplinary team delivering therapeutic interventions that empower patients and their families.

Main duties of the job

We are looking for someone who will:

  • Assess and manage a clinical caseload of palliative care patients across hospice services.
  • Design and lead patient-focused therapeutic groups to improve physical function and quality of life.
  • Collaborate with Allied Health Professionals and external services to ensure seamless, integrated care.
  • Provide education and training to staff and stakeholders on palliative rehabilitation.
  • Contribute to service development and quality improvement initiatives.

Our services include 24/7 care, Day Therapy Centres offering patient care and bereavement support, and Inpatient Unit in Llandudno, providing symptom control, respite, and end-of-life care.

PURPOSE OF THE ROLE:This Physiotherapist role is central to delivering high-quality rehabilitative care to patientswith life-limiting illnesses at St Davids Hospice.As part of the multidisciplinary clinical team, you will lead therapeutic interventions thatempower patients and their families.Your work will directly impact patient outcomes and help shape the future of ourrehabilitative services as we continue to grow and expand.

DIMENSIONS & LIMITS OF AUTHORITY:The role involves direct supervision of assistant therapy staff and volunteers whereappropriate.Ensure compliance with regulatory requirements related to palliative care rehabilitationand healthcare standards.Manage resources efficiently and within the allocated departmental budget for therapyservices.

MAIN DUTIES AND RESPONSIBILITIES:

Patient Care & Rehabilitation

  • Assess and manage a clinical caseload of palliative care patients across all Hospiceservices.
  • Develop and lead patient-focused therapeutic groups aimed at improving physicalfunction and quality of life.
  • Coordinate patient care, providing seamless access to ongoing support forpatients, families, and professionals.
  • Conduct holistic needs assessments, planning, and evaluating individualised careand rehabilitation programs.
  • Ensure patients and their families are fully informed about all availablerehabilitative services and resources.

Collaboration & Multidisciplinary Team Working

  • Collaborate with the wider Allied Health Professionals (AHP) team, includingoccupational therapy, counselling, and complementary therapies, to provideintegrated care.
  • Act as a patient advocate in multidisciplinary meetings, representing the palliativerehabilitation perspective.
  • Liaise with statutory and third-sector OT and physiotherapy services to ensurecontinuity of care.

Education & Service Development

  • Deliver specialist education and training to internal staff, healthcare professionals,and external stakeholders involved in palliative rehabilitation.
  • Contribute to the development and promotion of rehabilitative services, ensuringadherence to clinical guidelines and best practices.
  • Participate in service reviews and quality audits to drive continuous improvement.

Leadership & Professional Development

  • Act as a role model, demonstrating high standards of palliative rehabilitative care.
  • Support the professional development of all staff and volunteers within your areaof responsibility.
  • Take personal responsibility for your ongoing professional development,maintaining up-to-date knowledge of palliative rehabilitation practices.
