Job summary

A healthcare organization in Greater London is seeking a dedicated Pharmacy Technician to support the medication management team. This part-time position offers flexibility and the chance to contribute to improving patient health. As a Pharmacy Technician, you will assist with medication queries and work collaboratively with clinical staff. Candidates should have a BTEC/NVQ level 3 or equivalent in pharmacy services and at least two years of relevant experience. Salary is £35,763 to £40,149 per annum pro rata.

Job description

Central 1 Primary Care Network (PCN) is looking for a dedicated Pharmacy Technician to join their Network. You will work within the pharmacy team contributing to the service delivery across the PCN.

Main duties of the job

The post holder will work under the supervision of the NetworkClinical Pharmacist to ensure the safe, accurate and timely supply ofprescribed medication to patients.

You will provide technical and administrativesupport to the Clinical Pharmacists and Clinicians.

To support the primary care team with queriesaround medication from patients; including face to face and telephonesupport to patients, primary and community staff.

Under the direction of the clinical pharmacists we expect that you will be able to work independently and manage your own workload. You will need excellent communication skills to be able to liaise with patients, primary care staff, community pharmacists and other partners.

You will be working with Central 1 PCN in Newham, workingclosely with Clinical Directors, GPs and Practice Managers as well as othermembers of a multi-disciplinary team (MDT).

Job responsibilities
  • Workwith the Network Clinical Pharmacist, NHC Senior Clinical Pharmacist,Community Pharmacists and Clinicians in accordance with agreed practiceprescribing standard operating procedures
  • To support theachievement of the practices prescribing Quality and Outcomes Frameworktargets.
  • Dealing with medicationqueries including acute medication requests, patient medication andprescription queries.
  • To update and maintainaccurate patient medication records on the practice clinical computersystem, including advice given and action taken.
  • To assist the NetworkClinical Pharmacist with data analysis of prescribing behaviour in linewith NICE guidance and local clinical commissioning directives.
  • To assist with thedevelopment and review of medicine audits.
  • Provide a contact forpatients and colleagues with medication queries.
  • Project work/prescribinginitiatives under the support of the Pharmacist.
  • Conduct medicationreconciliation following discharge from hospital, clinic visits and othertransfers of care (e.g. new patients, care home residents), to ensure thatmedication changes are safely implemented in a timely fashion.
  • Liaise with the widerhealth care team including primary care staff, community nurses and carehome staff with medication queries.
  • To undertake regularaudits using different tools, for example to identify priority patientsfor medication review.
  • To support the safe andeffective repeat prescribing of high-risk medicines ensuring that regularmonitoring as per local/shared care guidance is taking place.
  • Supporting patients tomanage their medications, for example; synchronisation of medications,reviewing patients suitability for monitored dosage systems, and settingup electronic repeat dispensing.
  • To document interventionsmade to patients medications in their health care records to ensure thereis a clear audit trail and changes are clearly accounted for.
  • To promote costeffective, safe, evidence-based prescribing in accordance with localformulary, medication optimisation strategy and national guidance.
  • Attend local meetingsseeking to improve access to medicines or repeat prescribing processes.
  • Seek advice from thepatients GP or practice lead GP as appropriate.
  • Ensurepatients and their families/carers views are taken into account in every stageof the decision making process

Person Specification
Qualifications
  • BTEC/NVQ level 3 or equivalent in pharmacy services
  • Enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)
Experience
  • 2 year experience in a hospital, community or primary care.
  • Knowledge of the principles of medication optimisation.
  • Good pharmacy knowledge including terminology.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improvement of prescribing.
  • Knowledge and understanding of pharmacy law and ethics and current legislation.
  • Understanding of current national policies, in particular those that impact on medicines management services.
  • Experience of medicines reconciliation.
  • Effective time management (Planning & Organising).
  • Understanding of community pharmacy systems.
  • Understanding of QOF.
