Job summary

We are seeking a Clinical Pharmacist with independent prescribing to join our team of pharmacists and other health care professionals. The role requires an IP qualification and supports the GP team with routine EPS workload using EMIS. Ideally the pharmacist works from one of our surgeries and has experience within a GP Practice.

Person Specification

Qualifications and Training – Essential

• Qualified from an approved 18‑month training pathway or equivalent and able to practice and prescribe safely and effectively in a primary care setting (e.g., CPPE Clinical Pharmacist training pathways).
• Qualified independent prescriber.

Desirable

• Experience working within a GP Practice.

Responsibilities

Provide primary support to General Practice staff with prescription and medication queries.

• Support the repeat prescription system (including EPS).
• Deal with acute prescription requests.
• Conduct structured medication reviews and medicines reconciliation on transfer of care.
• Support safer prescribing systems and provide clinical medicines advice to patients within our single practice PCN.
