Job Summary

We are expanding our Pharmacy Team at Stafford Central PCN and are looking for a dynamic Clinical Pharmacist to join our team. The post is part‑time (30 hrs per week) and is an in house role at Gnosall Health Centre.

The post holder will work as part of a multi‑disciplinary team, manage targeted areas of chronic disease management, undertake structured medication reviews, support general practice staff with prescription queries, and promote safer prescribing practices.

Key Responsibilities
  • Review repeat prescribing policies and reauthorise requests, ensuring appropriate monitoring tests are in place.
  • Conduct level‑3 medication reviews to reduce inappropriate poly‑pharmacy and address public health and social care needs.
  • Reconcile medicines post‑hospital discharge and collaborate with community pharmacists to ensure patients receive necessary medicines.
  • Contribute to reductions in medicine‑related hospital admissions and readmissions by supporting optimal medicine use.
  • Interface with community and hospital pharmacy colleagues to develop referral processes and promote repeat dispensing services.
  • Manage practice formularies to improve quality, safety and cost‑effectiveness of prescribing.
  • Implement drug withdrawals, alerts and safety measures (e.g., MHRA actions).
  • Support the implementation of NICE and other evidence‑based guidelines.
  • Participate in clinics where medicines are the main intervention (e.g., anticoagulant monitoring).<\/li>
  • Lead multi‑morbidity and multi‑disciplinary reviews and provide education and training to the practice team.
  • Act as a source of medicines information and provide tailored recommendations for nurses or GPs on prescribing and monitoring.\
  • Undertake domiciliary and care home medication reviews, producing recommendations for staff.\
Qualifications
  • MPharm (4 years) or equivalent.
  • Membership of the General Pharmaceutical Council of GB (1 year pre‑registration training).
  • Prescribing qualification.
  • Postgraduate qualification in clinical pharmacy (1–2 years) or equivalent experience.
  • Maintains clinical and professional competence and CPD.
Desirable Qualifications
  • Membership of the Royal Pharmaceutical Council of GB.
Knowledge
  • Advanced knowledge of the clinical use of drugs.
  • Good knowledge of the pathophysiology of common disease states.
  • Good understanding of medicine management issues.
  • Thorough knowledge of the law and ethics relevant to medicines.
  • Knowledge of key drivers within the NHS.
  • Understanding of biochemical and haematological data.
Experience
  • Experience in clinical pharmacy.
  • Experience working with other healthcare professionals.
  • Experience of clinical audit.
  • Experience of community and hospital pharmacy.
Desirable Experience
  • Experience working within a PCN in primary care.
  • Minimum 5 years post‑registration experience with at least 2 years in primary care.
  • NHS Smoking Cessation Advisor.
  • Experience teaching and training other healthcare staff.
  • Awareness of financial management or budgetary reporting on medicines.
  • Experience providing evaluated drug information.
Skills
  • Ability to apply clinical knowledge appropriately in a variety of practice settings or patient groups.
