Central North Leeds PCN are looking to recruit to their pharmacy team, to continue delivering excellent pharmacy-led services to our population. We are looking for a motivated and aspirational clinical pharmacist to join our established team of pharmacists and pharmacy technicians, to work closely with the multidisciplinary PCN teams and the wider locality. The role is critical in supporting the delivery of the PCN Contract DES specifications and will help lead improvements to maximise safe, evidenced-based best practice to improve the quality of patient care.
Applications encouraged from pharmacists with patient-facing experience from any sector and/or pharmacists looking for a new direction. Post is not suitable for a newly qualified pharmacist. Interest in working in general practice is essential. As a PCN Pharmacist you will receive fully accredited training from CPPE and will be provided with appropriate supervision from a GP clinical supervisor and Senior Pharmacists.

We will support you to develop your role, including enrolment on the Primary Care Pharmacy Education Pathway from CPPE and becoming an independent prescriber. You will have line-management from the Senior Clinical Pharmacist within the PCN, and a GP clinical supervisor. Peer support is vital for this role and this will be provided by pharmacy colleagues within the PCN and wider system across Leeds.
We are a well-established and innovativePCN of 5 general practices with a long history of working together and workingwith a range of local providers, including across primary care, communityservices, social care and the voluntary sector, to offer more personalised,coordinated health and social care to around 80,000 people.
The PCN works hand-in-hand with generalpractice staff and employs a wide variety of roles outside of the pharmacyteam, namely; social and well-being prescribers, paramedics, occupationaltherapists, health and wellbeing coaches, nurse associates, careassistants and a phlebotomist. The PCN team work closely with each other healthcare professionals and meet monthly as part of a multi-disciplinary team todiscuss complex patients. There is a strong emphasis on learning anddevelopment of PCN staff and as such the PCN have created a supportiveenvironment for staff to flourish.
Job Purpose
Tosupport a Primary Care Network (PCN) within Leeds in all aspects of medicinesoptimisation.
Thepost holder will work within their clinical competencies as part of amultidisciplinary team to provide expertise in clinical medicines optimisationacross the PCN. This will include medication reviews with direct patientcontact and may include contributing to:
Management oflong term conditions
Management ofmedicines on transfer of care
Reviewing systemsfor safer prescribing
Contributing torepeat prescription authorisations and reauthorisation.
Addressing boththe public health and social care needs of patients
Contributing toachievement of PCN DES contract, QOF and locally commissioned qualityimprovement schemes.
Undertakingclinical audit
Developing,promoting and implementing the high quality, evidence based and cost effectiveuse of medicines in primary care in a way that maximises the benefits ofmedicines to patients and service users and which minimises risks (clinical,legal and financial).
Thespecifics of the role will be worked up between the post-holder and the PCN.
Thepost holder will be supported by existing pharmacy professionals employedwithin the PCN and a GP clinical supervisor. They will also engage with otherpharmacy colleagues in Leeds for peer support.
Thepost holder will be required to enrol on the Primary Care Pharmacy EducationPathway from CPPE, following which will be required to enrol on a prescribingcourse.
