The Patient Safety Investigator will provide high‑level support to the Patient Safety Manager, working within the Patient Safety team as part of the Clinical Governance structure within the hospital. The successful applicant will be responsible for assisting, guiding and supporting staff members with investigations into patient safety events, to align with The London Clinic's PSIRF policy and plan.

Key Duties
- Lead and support investigations that are timely, proportionate and aligned with PSIRF
- Apply systems‑based approaches and human factors principles to identify contributory factors
- Gather and analyse evidence, including statements, interviews and clinical records
- Produce structured investigation reports that offer clarity, assurance and learning
- Review Datix data to identify themes and support improvement planning
- Support clinical teams with monitoring and delivery of improvement actions
- Contribute to patient safety education across the organisation
- Engage with patients, families and regulators with professionalism and sensitivity
Responsibilities
- Assist staff members with timely, comprehensive investigations of reported patient safety events, coordinating 72‑hour review meetings and allocating incidents to the appropriate area and team.
- Conduct thorough investigations into patient safety incidents using approved methodologies (e.g. systems‑based approaches, human factors analysis).
- Produce high‑quality investigation reports with clear findings, recommendations, and improvement actions.
- Work closely with the Patient Safety Manager, discussing patient safety incidents and updating as appropriate through each stage of the process.
- Gather and analyse medical records, staff interviews and other relevant evidence pertinent to all safety incidents.
Ideal Candidate / Qualifications
- Registered clinician with a UK professional body such as NMC, HCPC or GMC.
- Current registration with a UK professional body.
- Clear knowledge of patient safety principles, the national patient safety framework and the fundamentals of clinical governance.
- Understanding of PSIRF and ideally experience supporting or delivering PSII investigations.
- Awareness of Datix processes and the use of incident reporting systems.
- Experience in patient safety, quality improvement or clinical governance within an acute healthcare setting.
- Relevant clinical experience in a hospital environment.
- Experience conducting or supporting investigations into patient safety incidents.
- Strong analytical skills and the ability to interpret data and identify patterns.
- Skilled in producing clear and concise reports for assurance purposes.
- Confident communicator with the ability to engage with patients and staff sensitively and professionally.
- Competent in the use of Microsoft applications including Word, Excel and PowerPoint.
Person Specification
- Ability to demonstrate awareness of Clinical Governance, Patient Safety and the standard of care expected in the NHS.
- Skills in managing patient interactions with empathy, compassion and professionalism.
- Experience in undertaking, or assisting in, investigations into safety incidents.
- Proven ability to manage interactions with patients, families and regulatory bodies sensitively.
- Excellent report‑writing for assurance experience.
