Job summary

NHS Professionals seeks a Patient Pathway Coordinator in Reading to manage the patient journey from referral to discharge within a multi-disciplinary team. You will supervise the PPC team and ensure smooth coordination of appointments, diagnostics, and communications across Trusts.

The role requires NHS experience, organisational skills, and proficiency with Word and Excel. It is a temporary 3-month post with a view to permanency, based at Craven Road, Reading.

Qualifications

  • NHS experience essential.
  • Vocational Level 3 qualification or 2+ years relevant experience.
  • Experience using MS Word and Excel.
  • Strong organizational and interpersonal skills.

Responsibilities

  • Process and log referrals and bookings per policy.
  • Schedule appointments and manage capacity efficiently.
  • Coordinate diagnostics and ensure results available for consultations.
  • Coordinate cross-hospital input for pathways.
  • Record clinic attendance outcomes in EPR.
  • Handle clinic cancellations and reschedule as targets.
  • Use medical transcription system for letters per SOP.
  • Liaise with records, clinicians and admin teams to ensure documents available.
  • Identify cancer pathway patients and liaise with Cancer Services MDT.

Job Title: Patient Pathway Coordinator
Location: Craven Road, Reading, RG1 5AN
Hours: 37.5 hours per week – Monday – Friday mixture of shifts 8am – 4pm & 9am – 5pm
Trust: Royal Berkshire Healthcare NHS Foundation Trust
Role type: Temporary for 3 months with possibility of becoming permanent

Role details

The post holder will be the point of contact for all administrative issues relating to patients’ pathway of care. The post holder will work as part of a multi-disciplinary team and will provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly. The post holder will supervise and allocate work to the PPC team.

Duties & responsibilities
  • Process and log all referrals, including paper and Choose and Book, in line with Trust Access & Administration Process Policy and speciality SOP
  • Schedule new and follow up appointments, ensuring that capacity is proactively and efficiently used. This includes booking patients into the right clinic to ensure that they are seen by the most appropriate clinician first time and for subsequent follow ups
  • Ensure that any diagnostics and investigations that are required as part of the patient pathway have been requested and that the results are available for consultations. This will include those that are carried out at other Trusts
  • Co-ordinate appointments and procedures at other hospitals and organisations, where the pathway requires input from these
  • Ensure outcomes of clinic attendance have been accurately recorded on EPR by the Patient Pathway Assistant (‘cashing-up’). This will need to be done for those for offsite clinics
  • Complete clinic cancellation forms and process according to SOP
  • Reschedule outpatient clinics as requested – to be done in line with waiting time targets
  • Use the Trust’s medical transcription system and ensure that letters are processed in accordance with SOP
  • Liaise with patient records staff, clinical colleagues, other admin teams and other organisations to ensure all medical notes, referral letters, results and discharge summaries are available for outpatient appointments and consultations
  • Recognise when patients are on cancer pathways and, liaise with Cancer Services MDT coordinators to ensure patients are handed over appropriately

This role may require you to show evidence of vocational level 3 qualification or at least 2 years relevant experience.

  2. Knowledge of the systems used, to effectively complete your role to the highest standard at all times
  3. A sense of teamwork, gained through working alongside and supporting colleagues from all levels, within the organisation
You’ll have the following skills/experience:
  • Ability to plan and prioritise own workload and that of the service on a daily basis, working mainly without supervision and using own initiative.
  • NHS experience (essential)
  • Experience of using Microsoft Office, including Word and Excel
  • Reliable and organised paying attention to detail, with excellent interpersonal skills
  • Experience of handling patient complaints
