A healthcare provider in Goole is seeking a motivated Patient Coordinator. The role involves managing patient appointments, ensuring efficient communication between patients and medical staff, and maintaining reception operations. You will primarily be responsible for managing appointments, facilitating communication between patients and health care providers and ensuring the smooth operation of our reception area. Patient Coordinator Role x 2 :- 25 hours per week
Various hours available Monday to Friday 8.00am to 6.30pm at both Ash Grove Medical Centre and Eggborough Surgery

Key responsibilities:
Welcome patients and visitors with a warm and professional manner at the reception front desk, marking their arrival on the computer, make appointments as requested, hand out prescription requests, pathology forms and deal with any queries that may arise.
Manage appointment scheduling, confirmations and reminders through our patient management system.
Handle patient inquiries and provide information about our services, procedures, and policies.
Coordinate communication between patients and medical staff, ensuring timely updates and follow ups.
Maintain accurate patient records and assist with data entry tasks.
Participate in team meetings and contribute to process improvements for better patient care.
Support the overall operations of the reception area, including managing phone calls, emails and our online consultation platform.
Care navigating patients to the most appropriate service
Ensure that confidentiality is maintained at all times
Take requests for home visits
Contact patients via telephone, text, email and letter to arrange any follow ups as requested by the clinicians or management
Carry out the partners instructions and requests
All staff are required to cover their colleagues and provide cover for annual leave and sickness
Candidates must have strong communication skills, be reliable, trustworthy and have a willingness to lead, the individual must be able to prioritise workload.
Previous experience in a healthcare setting or similar role is preferred
Ash Grove Medical Centre is CQC rated 'Good' across all five areas. We are a large GP Practice with 13,000 patients based in Knottingley with a branch surgery in Eggborough.
Ash Grove Medical Centre is led by four GP Partners working along side the Practice Manager.
Our clinical team is supported by a Salaried GP, three Advanced Clinical Practitioners, three Practice Nurses and three Health Care assistants.
We have a large non clinical team of patient coordinators, administrative and prescription administrators.
The patient coordinator will be the focal point of communication between the patients, doctors and other clinical and non-clinical staff. This encompasses dealing with incoming calls from external organisations, the general public and patients; dealing with queries, appointments and reception desk cover. The role also has responsibility for dealing with prescriptions, GP liaison and instructions.
