Qualifications

Responsibilities

• Coordinating Learning Disability reviews and ensuring reasonable adjustments.
• Managing recalls for Depression and Dementia Reviews.
• Monitoring patients on NOAC/DOAC therapy.
• Overseeing injection therapy schedules and safety requirements.
• Contacting Bowel Screening non-responders.
• Supporting delivery of NHS Health Checks.
• Reviewing Childhood Immunisation missed appointments.
• Coordinating Cancer Care Reviews.
• Supporting carers with resources.
• Updating the practice website.
• Assisting with Research projects.
• Main contact for local residential home coordination.
