Job responsibilities

The Primary role of the Advanced Paramedic Practitioner may vary but it has a basic remit of the delivery of care to manage patients presenting to the practice by the right healthcare professional. They will act within their professional boundaries to manage the presenting patient - from initial history taking, clinical assessment, diagnosis, treatment and evaluation for care. They will demonstrate critical thinking in the clinical decision making process and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. He/She will carry continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care also setting standards of care within the practice.

Take a lead role in assessing, diagnosing, planning, implementation and evaluating care of patients within the practice population.

Clinically examine and assess patients needs from a physiological and psychological perspective and plan clinical care accordingly.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition.

Diagnose and manage both acute and chronic conditions integrating both drug and non drug based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence based practice and national and practice protocols, and within scope of the practice.

  1. To provide assessment, diagnosis and treatment at first point of contact by attending to patients according to patients needs either in the practice sites or patients homes/care homes.
  2. To accurately triage and prioritise patients, including in emergency situations.
  3. To evaluate clinical information from examination and history taking and initiate appropriate definitive treatment.
  4. To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis.
  5. To signpost appropriately to other services in the identified healthcare pathway.
  6. To discharge or treat, as appropriate and within the scope of practice.
  7. To maintain accurate and systematic documentation of patient interactions.
  8. To assist with the daily duty list and on call triage.
  9. To advise and support members of the public on healthy lifestyles, health promotion and disease prevention.
  10. To provide clinical advice to other members of the team where appropriate.
  11. Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.
  12. Initiate and interpret tests and investigations as appropriate.

Fairfield Surgery is an 8,300 patient GMS practice based in the Port Talbot Resource Centre with a friendly administrative team. There are 3 GP Partners with a clinical team of Practice Pharmacist, Practice Paramedic, Practice Nurses and a Health Care Worker. Supported by a community team of District Nurses, Midwives and Health Visitors.
