Job Summary

We are looking for highly motivated and pro‑active Nurses to join our Onward Care Team at Worcestershire Royal Hospital / Alexandra Hospital Redditch to ensure patients get the care that they need, when they are ready to leave hospital, ideally at home first.

The role will involve working closely with patients, families and the acute hospitals nursing, therapy and medical teams and social care colleagues in a multi‑disciplinary, inter‑disciplinary way to ensure a safe and timely transfer with our community discharged to assess providers.

You will be expected to be organised, with excellent communication and IT skills. You will need to be able to work under pressure, prioritising your workload against constantly changing demands. If you can think innovatively and use your own initiative and judgement to resolve complex issues, this is the opportunity for you.

You will be a key part of our developing team that strives to ensure all patients achieve a safe discharge at the right time, to the right place, first time.

The hours will be worked over 7 days, to include weekends and Bank holidays. Can offer full or part-time working.

We provide a 7‑day service across two sites at Alexandra Hospital and Worcester Royal Hospital. You will be expected to be able to travel to meet the needs of the service as this role may require cross‑site working.

Main Duties of the Job

Work collaboratively within the Acute Hospitals which includes the Emergency Departments, assessment units and all wards to proactively identify patients who will need Onward care on discharge/transfer and to support the multi‑disciplinary team with facilitating complex discharged aiming for home first.

Take the lead in discharge arrangements for individual patients where appropriate liaising with all members of the multi‑disciplinary teams in the acute setting and providers in the community.

Act as a resource for patients, relatives and carers requiring information on discharge processes, services available and all other issues relating to discharge and admission avoidance in accordance with current legislation.

Adopt a Home first approach, enabling patients to return safely home earlier from hospital.

Assess patients for suitable and safe transfer to community hospitals, community nursing teams and community provider services, both in and out of county.
