Overview

Are you a passionate and caring Registered Mental Health Nurse looking for a rewarding career with excellent training and opportunities for development?

Supporting Older Adults within the Bolton community, who are experiencing a range of complex mental health issues. We work closely with families, carers and other health and social care professionals to promote recovery and autonomy.

We are a warm and friendly Multi-Disciplinary Team with a strong focus on staff wellbeing and development. To support staff wellbeing we offer one day a week remote working, alongside regular team events. Within the team we have monthly CPD sessions in additional to regular Line Management and Clinical Supervision, alongside peer support. We promote reflective practice and encourage practitioner development.

Responsibilities
  • Fulfil the role of Care Coordinator and the requirements of this role.
  • Support a caseload of older adults with mental health problems, using evidence-based, client-centered principles to assess, plan, implement, evaluate, and record interventions.
  • Respecting individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
  • Provide support and advice to carers.
  • Participate in the duty rota.
  • Provide a nursing perspective within the multi-disciplinary team.
  • Maintain nursing standards and contribute to the development of nursing practice in the team.
  • Contribute within the wider MDT in the development of clear care pathways regarding support, care and treatment of older adults with mental health problems.
  • Support student placements.
  • Work weekends and bank holidays on a rota basis, typically one weekend in every 12 weeks.
