Responsibilities
  • To provide a person centered approach and promote independence and empowerment to support people to attain their full potential
  • To enable clients to achieve or regain maximum skills and wellbeing for independent living in the community.
  • To focus on a restorative approach to maximizing an individual’s independence and enable them to link back into their community and participate in meaningful social roles.
  • To support people to maximize their abilities to manage independently using a range of evidence based assessments, and interventions including; helping people compensating for lost abilities, learning or re-learning the skills necessary for daily living, restoring body functions, providing advice and education to support workers.
  • To supervise designated Reablement staff
  • To provide on- going education and training of Reablement staff
  • To evaluate effectiveness of interventions

  • To be professionally and legally responsible and accountable for all aspects of work in line with the College of Occupational Therapists Code of Conduct, and Health Professions Council Registration requirements.
  • To ensure a high standard of practice and to support more junior staff to do likewise.
  • To independently manage a caseload of potentially complex clients where there are multiple conditions referred to the team, working as an autonomous practitioner and making own clinical decisions, within appropriate professional guidelines.
  • Undertake assessment of function and ability taking account of the priorities of the client demands of the environmental, analysis of activities, physical, psychological, social circumstances and needs.
  • To analyse the results of the assessment, considering of a range of options, using clinical reasoning and professional judgment and communicating these to clients in order to reach a decision, in collaboration with the client about how to meet those needs.
  • Develop individual outcome focused on reablement goals, select the most appropriate approaches, techniques and methods of intervention, and produce programme plans in collaboration with the client, thus enabling clients to regain independence and wellbeing and reable them back into, or remain living within their own home or community.
  • Adopt a problem‑solving approach to intervention and use appropriate task analysis, teaching approaches, facilitating techniques, compensatory techniques, equipment and adaptations to overcome barriers to independence.
