Job description

A Vacancy at East Suffolk and North Essex NHS Foundation Trust.

Overview

We are looking for an experienced, motivated clinician to join our team. You will be a registered Occupational Therapist / Senior Physiotherapist with demonstrable expertise in discharge planning, supporting patients to become experts in their own care and to remain in their own homes.

It is expected that you will have excellent communication skills, enjoy working with a variety of service users of different age groups, have good self-management skills and an ability to prioritise your case load. You should ideally have experience of working in a variety of clinical settings with sound medical, respiratory and social knowledge.

Working Pattern

This post has 7 day working, in a 4 week pattern, with x2 days off prior to or after the weekend. Periods of requiring you to work 10:00-18:00 Mon-Fri once every 8 weeks.

Responsibilities

• Provide rapid assessments in a fast-paced environment.
• Formulate robust discharge plans.
• Work with the wider MDT to problem solve and support patients into their preferred place of care.
• Lead planning, coordinating, delivering and evaluating the care provided.
• Supervise junior staff and students.
• Build networks and relationships across several different clinical teams.
• Embrace innovation and integrated working and commit to delivering excellence in care alongside other OTs, Physiotherapists, Associate Practitioners and Assistant Practitioners.

Qualifications & Experience

Required: Registered Occupational Therapist / Senior Physiotherapist with expertise in discharge planning and experience in diverse clinical settings.

Preferred: Experience in managing a caseload, supervising junior staff, and working within multidisciplinary teams.
