Job description
Overview

We take referrals for medical and/or social crisis from the community and (e.g. NWAS, GPS) and SRFT departments, aiming to promote early discharge from hospital and avoid unnecessary hospital admissions by implementing clinical treatment/care plans in the patient's own homes in the Salford area.

Responsibilities
  • Deliver high quality care in the community, performing holistic assessments, planning, implementing, and reviewing care provision across the Virtual Wards pathways of care.
  • Manage acutely unwell patients at home using a variety of pathways, including IV antibiotics via the cellulitis pathway and the DVT pathway, and develop further pathways to manage patients in their own environment and prevent unnecessary admissions.
  • Work within the Hospital and Care at Home (H&C@H) virtual wards model of care, utilising technology and new ways of working to provide a safe alternative to hospital admission and support people to be discharged from hospital earlier.
  • Collaborate with a multidisciplinary team of clinicians, including Advanced Practitioners, Allied Health Professionals, and support staff, to provide high quality comprehensive care to patients who require step down from hospital and deliver clinical interventions in the home environment to prevent hospital admissions.
  • Maintain close working relationships with NWAS and GPs.
Qualifications
  • Current NMC Registration.
  • Preferably 2 years of community experience and a clinically based nursing degree.
  • Car user is essential to this role.
