A healthcare provider in Salford is seeking a dedicated community nurse to deliver high quality care at home and manage acutely unwell patients. This role involves performing holistic assessments, developing care plans, and collaborating with multidisciplinary teams to prevent unnecessary hospital admissions. Candidates should have current NMC registration, a clinically based nursing degree, and a minimum of 2 years community experience. We take referrals for medical and/or social crisis from the community and (e.g. NWAS, GPS) and SRFT departments, aiming to promote early discharge from hospital and avoid unnecessary hospital admissions by implementing clinical treatment/care plans in the patient's own homes in the Salford area.
