MentalHealthNurse – Primary Care / General Practice

Hours: Up to full-time (flexible options available)

Location: Lovemead Group Practice, Roundstone Surgery, Trowbridge

Lovemead Group Practice is looking for a compassionate and experiencedMentalHealthNurseto join our multidisciplinary team in primary care. This is an exciting opportunity to use yourmentalhealthexpertise at the heart of the community, working directly with patients in a trusted GP environment.

You’ll play a key role in helping patients manage theirmentalhealthneeds early, offering timely assessments, short-term interventions, and onward referrals – reducing pressure on secondary services and improving access to local support.

Responsibilities
  • Providing urgent support and crisis management to patients presenting withmentalhealthconcerns
  • Undertaking comprehensive assessments, developing person-centred care and safety plans
  • Delivering brief, evidence-based interventions and follow-up support
  • Supporting GPs and the wider clinical team with advice, signposting and joint management of complex cases
  • Building strong links with communitymentalhealthteams, IAPT services, and voluntary sector organisations to ensure seamless patient care
About you
  • RegisteredMentalHealthNurse(RMN) with current NMC registration
  • Experienced in supporting adults with common and severementalhealthneeds, ideally within community or primary care settings
  • Previous experience in a general practice or primary care environment is highly desirable, or a strong interest in developing this area of practice
  • Confident working autonomously, while also being an active part of a collaborative, patient-focused team
  • Compassionate, approachable, and able to build trusting relationships quickly
  • Skilled in risk assessment, crisis management, and safety planning
  • Able to demonstrate excellent communication and documentation skills, with a proactive and organised approach to patient care
