Overview
We are seeking highly motivated Specialist Community Public Health Nurses to join our 0-19 Healthy Child Programme in Norfolk. This role involves leading delivery of our Healthy Child Programme, implementing planned programmes of care, evaluating care delivered, and liaising with other agencies.
Responsibilities
- Work as part of a multidisciplinary 0-19 team to identify the health needs of children and young people, their families and the community and contribute to developing a strategy to address identified needs.
- Support the 0-19 service offer and undertake migration training within 2 years of employment into the other SCPHN pathway (e.g., Health visitor to school nurse).
- Use knowledge and skills to assess, plan, implement and evaluate packages of care.
- Be competent as a non‑medical prescriber.
- Support families through a range of methods including home visits, digital drop‑in clinics and video contacts to assess their needs.
- Support children and young people and their families with emotional and behavioural issues.
- Advise their client group on the promotion of health and the prevention of illness.
- Link practice with local and national targets and deliver public‑health programmes based on current priorities and targets.
- Plan evidence‑based health promotion programmes, order resources, compile teaching plans with aims, objectives and outcomes, and evaluate the sessions.
- Act as a health link for other agencies (Children's Services, GPs, Midwifery Services, voluntary services, public and private agencies) and work closely with partner agencies.
- Work with the Locality Leadership Team to support children, young people and families considered vulnerable or at risk, adhere to current Safeguarding Procedures and participate in child protection case conferences and case review meetings.
- Contribute to development of local policies and procedures relevant to your area and to relevant service‑design projects.
- Take responsibility for your own clinical practice including clinical supervision and adhere to the NMC code of conduct.
- Support service delivery at the Single Point of Access in Just One Number and support other localities to ensure that all relevant clinical skills are kept up to date.
- Promote early intervention and the use of digital resources via Just One Number and Just One Norfolk to enhance self‑care skills for children, young people and their families.
- Confidently utilise virtual platforms in assessments, interventions, training and networking to support effective practice and partnership working.
- Communicate complex and sensitive information to parents/carers and young people; communicate within the multi‑disciplinary team and model effective team working and communication skills.
- Develop links with Early Childhood and Family Services, educational settings and other agencies involved in promoting children’s well‑being.
- Act as first line response to complaints working within national and corporate guidelines.
- Contribute to the training of student nurses and other disciplines during community experience programmes.
- Contribute to ensuring that CCS responsibilities to engage and consult with patients and the public are met in line with statutory requirements.
- Demonstrate responsible use of resources with an awareness of budgetary restraints.
- Ensure health monitoring equipment is operating to a high standard, maintained and calibrated.
- Delegate work to other members of the team, ensuring clarity regarding feedback mechanisms.
- Contribute to clinical audit and research, highlighting unmet needs within the practice population and maintaining the Health Profile of the practice population.
- Observe and maintain strict confidentiality in line with the Data Protection Act and Trust information governance policies.
- Participate in clinical and other audits, clinical supervision and emergency preparedness processes as required.
Additional Information
Travel: The role requires independent travel on a daily basis across Norfolk (both rural and urban). You must hold a full and current driving licence and have access to your own transport.
