Qualifications

  • Experience in undertaking Social Care and Mental Capacity Act assessments.
  • Ability to complete Safeguarding enquiries and care support planning.
  • Inquisitive and evidence-based recording with strength-based practice focus.

Responsibilities

  • Undertake assessments to achieve better outcomes for adults supported.
  • Work in partnership with service users and informal carers.
  • Provide personalized solutions considering community resources.
