A healthcare provider in Scotland is seeking a Specialist Nurse for the Integrated Front Door team. This role involves supporting direct admissions and virtual ward patient care. Candidates should possess excellent communication skills and demonstrate a commitment to patient care, team collaboration, and service development. A driving licence is required. The position is permanent with a salary range of £41,608 - £50,702 plus additional allowances.
We have an exciting opportunity for Specialist Nurses to join an evolving Integrated Front Door team.
These posts will be based primarily within Hospital at Home supporting direct admission and the care of patients in the virtual ward. There is a vision to expand the service in line with Scottish Government initiatives. Applicants should be committed to providing a high standard of patient care and interact well within the multi-disciplinary Team.
Applicants should have good communication skills, and a commitment to developing change and promote innovation within the front door services. Post holders are required to hold a driving licence.
The ability, drive and commitment to work as part of a team, sometimes at a distance, and to contribute to the local development of primary care and acute services is important.
