  • Work within a multi-disciplinary team to deliver a nurse‑led service and prevent hospital admissions.
  • Lead the development of individualised patient care plans, while collaboratively working alongside the inpatient team and championing clinical excellence.
  • Support patients daily to achieve their activities of daily living, ensuring a best quality of life possible, with collaborative delivery.
  • Ensure quality care planning and continuity of care by improving relationships and communication with partners and other services.
  • Deliver non‑judgemental care that makes a real difference, as defined by our bespoke Health in Justice competency framework.

Position Details

This is a full time position. You’ll receive an annual salary up to £40,211 per annum FTE, depending on experience.
