Overview

PCCO PCN are looking for an experienced advanced clinical practitioner to work within the 'Team Up' team to provide both proactive and acute home visits for housebound patients. Our PCN has approximately 46,500 patients with a wide range of demographics, including housebound patients, those living within high levels of deprivation and people who may be living with long term conditions at a young age. We are looking for an enthusiastic individual who is willing to contribute to the management of complex long-term conditions and who has a collaborative approach which seeks to work with wider healthcare, community and social teams within our neighbourhood. The role involves autonomous clinical assessments, diagnosis, and treatment planning in patients homes, managing acute and long-term conditions, and collaborating closely with the wider nursing and multidisciplinary teams. The position supports urgent care pathways, enhanced care in community settings, and proactive risk stratification to prevent future incidents. Flexibility is offered in terms of full-time or part-time hours, with a focus on delivering high-quality, patient-centred care in the community.

Responsibilities
  • Conduct autonomous assessments, diagnoses, and treatment plans for patients in their homes.
  • Manage acute presentations, palliative care needs alongside our palliative care nurse, and long-term conditions in conjunction with our wider nursing team.
  • Prioritise patients including urgent situations, demonstrating the use of clinical assessment skills to obtain history, patient examination, diagnosis and treatment, including on-medical prescribing where appropriate.
  • Evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.
  • Undertake and interpret diagnostics and take appropriate action to manage patient care.
  • Signpost appropriately to other services in the identified healthcare pathway.
  • Refer patients to appropriate care settings within locally agreed healthcare pathways.
  • Discharge or treat, as appropriate and within scope of practice.
  • Prescribe medications and refer to appropriate services as required.
  • Lead on frailty assessments and proactive care planning.
  • Participate in MD huddles and care reviews.
  • Proactively use digital tools and searches to risk stratify patients alongside our clinical management team and to plan and carry out proactive visits to prevent future incidents, illness and falls.
  • Respond to urgent visit request and contribute to same-day care pathways.
