A dedicated health service provider in Doncaster is seeking a Band 5 Home Treatment Practitioner to provide intensive support for individuals with acute mental health needs. This role involves triaging referrals, delivering nursing care, and collaborating within a multi-disciplinary team. The position requires a nursing degree and relevant mental health experience. Shift work is required, offering a balance of work and life.
Band 5 Home Treatment Practitioner, 37.5 hours – 08:00 to 21:30, 365 days a year. An exciting opportunity has arisen for a band 5 nurse to join Doncaster intensive Home Treatment Team. The service provides short-term intensive support in the community to individuals with acute mental health needs to prevent hospital admission.
Rotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) employs around 4000 colleagues who are valued and respected. The Trust has a wide portfolio including mental health, physical health, learning disability and drug and alcohol services.
We provide a service between 08:00 and 21:30 seven days a week and staff are expected to work shift patterns to cover this. Most team members work 3 long shifts to support a good work‑life balance. The team works in partnership with third sector and peer organisations to deliver person-centred crisis care; transparency and openness are vital.
