A large integrated health care organisation in the UK is seeking a Health Care Assistant to work autonomously within the Out of Hours Urgent Community Response Team. The role involves assisting qualified nursing staff in delivering care, showcasing good problem-solving skills and adaptability. A full UK driving license and vehicle access is essential for this position. Training and development support is provided.
The newly formed Out of Hours Urgent Community Response (OOH UCR) team combines the patients traditionally seen by both District Nursing and our Intensive Home Support Service. This is an exciting time to join the service as we come together to enhance the level and breadth of care and treatment we are able to deliver to our patients in their own home, preventing unnecessary hospital admissions and meeting the national guidance around Urgent Community Responses. The role involves working autonomously with the support of the wider team to deliver planned care, and successful candidates will be able to demonstrate good problem-solving skills, adaptability and resilience at interview. Training and development support will be provided with a dedicated mentor and competency framework in place to ensure that the post holder is secure and confident in their role. Operating across our two bases at Burnley General Hospital and St Ives House, Blackburn, you will need to be able to problem solve and adapt to change, know when to seek support and guidance, and contribute to the delivery of safe, personal and effective care.
Full UK driving license with access to a vehicle is essential.
