Job description

This is an exciting opportunity to join the Bee at Home team as a Health and Social Care Assessor. This is a multi skilled role where you can expect to support with coordinating shifts, triaging patient referrals and carrying out assessments in patients own homes across Hull and East Riding. The shifts are 8am-8pm and 10am-10pm on a rota basis.

Job responsibilities

Person Specification
Qualifications
  • Level 4 in Health and Social Care or equivalent level of knowledge, skills and experience
  • Care Certificate or willingness to undertake this within induction period
Knowledge
  • Knowledge of planning and completing initial client assessments
  • Knowledge of policies, procedures and practices relating to domiciliary care
  • Knowledge of a range of software programmes including Microsoft Word, Excel and Outlook
  • Knowledge of completing risk assessments appropriate to domiciliary care
  • Knowledge of relevant legislation e.g. Mental Health Act (1983), Mental Capacity Act (2005), Safeguarding Adults and Deprivation of Liberty (DOLS)
  • Clear understanding of domiciliary care and client needs in the community
  • Knowledge of databases used within domiciliary care
Experience
  • Experience of completing initial client assessments either in the community or within a residential setting
  • Experience of working in a health and/or social care setting
  • Experience of undertaking risk assessments
  • Experience of keeping accurate records and inputting data into relevant database systems
  • Experience of undertaking a similar role within domiciliary care services
  • Experience of working with people with a range of health conditions such as dementia
  • Experience of working with people with end of life needs
  • Experience of liaising and working with multidisciplinary teams or care teams
