A healthcare organization in Hull is looking for a Health and Social Care Assessor to support coordinating shifts and conducting assessments in patients' homes. This is an exciting opportunity to join the Bee at Home team as a Health and Social Care Assessor. This is a multi skilled role where you can expect to support with coordinating shifts, triaging patient referrals and carrying out assessments in patients own homes across Hull and East Riding. The shifts are 8am-8pm and 10am-10pm on a rota basis.
