Job description

We are seeking a well-organised and detail-focused person to become part of our welcoming general practice team. The role involves maintaining accurate, up-to-date patient records that meet both clinical and contractual standards. Using the EMIS Web system, you’ll be responsible for SNOMED coding, medical record summarisation, and helping ensure information flows smoothly across the practice. If you enjoy working with data, have strong attention to detail, and want to contribute to patient care behind the scenes, this position may be ideal for you.

Overview

Role: Medical Summariser / Medical Records Administrator (EMIS Web). Focus on accurate coding, summarising patient histories, and preparing records for online access and clinical use.

Responsibilities
  • Ensure accurate SNOMED coding, covering QOF, IIF, and Enhanced Services, to facilitate patient recalls and maintain comprehensive medical records.
  • Summarise the medical history of all new patients, both from paper files and electronic sources.
  • Prepare and summarise patient records for anyone requesting Online Access before access is provided.
  • Open, organise, scan, and code all incoming correspondence, including referral letters and confidential paperwork.
  • Manage clinical correspondence by directing it to the correct departments.
  • Process Out-of-Hours documentation and prepare it for review by clinicians.
  • Chase up incomplete medical records and contact external practices when further details are required.
  • Keep existing patient files accurate and up to date, making changes to diary entries where necessary.
  • Monitor the GP2GP inbox and ensure records are prepared for scanning and summarisation.
  • Undertake assigned duties and provide support with any other reasonable tasks as needed.
  • Process repeat medication requests and related paperwork as required.
