About the Role:

Surbiton PCN LTD are looking for a proactive and confident part-time GP Assistant to join our friendly team. The successful candidate will work at Brunswick Surgery for 12 hours per week (Surbiton Health Centre) as part of a job share and will play a key role in supporting the management of patients with long‑term conditions, including hypertension, diabetes, and other chronic illnesses.

Brunswick is an established training practice for VTS/medical students. The practice is increasing its list size, and currently has 9,100 registered patients. The current GP clinical team consists of 4 Partners, 3 Part‑time Salaried GPs and 1 Trainee. They are supported by a friendly multi‑disciplinary and non‑clinical team. Brunswick utilise EMIS Web clinical system and have an excellent QOF achievement.

Key Responsibilities:
  • Assist in the coordination and management of patients with long‑term conditions.
  • Book patients for reviews with GPs and the nursing team.
  • Ensure timely follow‑up and accurate record‑keeping.
  • Provide excellent patient support and communication throughout their care journey.
Essential Requirements:
  • Background knowledge of hypertension, diabetes, and other long‑term conditions.
  • Strong interpersonal skills with the ability to build rapport with patients and colleagues.
  • Confident, organised, and able to work independently as well as part of a team.
  • Good IT skills and attention to detail.
Desirable:
  • Previous experience in a healthcare setting.
  • Understanding of primary care processes and patient pathways.
