Main Duties of the Job
  • Deliver a comprehensive clinical pharmacy service to the gastroenterology ward on a rotational basis.
  • Support and enhance specialist pharmaceutical care services for patients receiving high-risk, high-cost medications, such as advanced therapies and hepatitis anti-virals, delivered via Homecare services.
  • Provide clinical pharmacy support to outpatients and endoscopy units, as well as the daycase unit.
  • Offer a ward-based clinical pharmacy service, including individual prescription reviews, medication history confirmation, and advice on dosage, side effects, cautions, and necessary monitoring.
  • Actively participate in ward rounds, clinical meetings, and outpatient clinics to provide pharmaceutical advice to prescribers and healthcare professionals.
