Job description
Overview

The vacancy is for approximately 24 hours a week, working over 3 days a week, including one shift within extended hours until 20:00. Flexibility is crucial to accommodate training, holidays and occasional sickness.


Howard House Surgery supports over 8,500 patients from diverse backgrounds. The team focuses on customer service and staff development to deliver excellent standards of patient care. Candidates with prior primary care experience are encouraged to apply.


We use SystmOne and AskMyGP to facilitate effective and efficient ways of working.


The Clinical Prescribing Practitioner will be the first point of contact for patients with all types of health problems. They will make decisions and apply appropriate interventions, in the patient’s home or at the surgery. Responsibilities include initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. The post-holder will work autonomously within Practice, NMC and HCPC policies, protocols and guidelines, demonstrating safe clinical decision-making, high levels of critical thinking and holistic practice. The role requires independent action while maintaining continual access to a GP for advice and support as necessary.


Responsibilities


  • Initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

  • Work autonomously within Practice and HCPC guidelines.

  • Act independently with continual access to a GP for advice and support.

  • Accurately examine and assess patients with an undifferentiated diagnosis from a physiological, psychological and social perspective.

  • Diagnose and manage both acute and chronic conditions, integrating drug and non‑drug treatment methods into a management plan.

  • Seek advice and support from a senior clinician as appropriate to inform decision‑making around patient care.

  • Prioritise health problems and intervene appropriately in urgent or emergency situations, including initiation of effective emergency care.

  • Review medication for therapeutic effectiveness and administer medication in accordance with PGD and Independent Prescribing.

  • Provide information and advice on prescribed or non‑prescription medication, including side‑effects and interactions.

  • Support patients in compliance with and adherence to prescribed treatments.

  • Ensure full and accurate documentation in patient notes of clinical history, physical examination, diagnosis and treatment plan.

  • Maintain a strong command of written and spoken English with an excellent telephone manner.

  • Demonstrate good time‑management skills and the ability to follow protocols or instructions with attention to detail.


Qualifications


  • Recent clinical experience in an acute or community setting (Registered Nurse, Independent Prescriber, or Registered Paramedic with HCPC).

  • Basic Life Support (BLS) or equivalent resuscitation qualification.

  • Valid driving licence and reliable vehicle access.

  • Excellent computer skills and ability to adapt to various systems; familiarity with AskMyGP and SystmOne is preferred but not essential.

  • Willingness to undergo Disclosure and Barring Service Check.

  • Full training will be provided.
