A leading healthcare organization in the UK is looking for an experienced Registered Nurse to join their Discharge Team at Wexham Park Hospital. The role involves coordinating complex discharges and ensuring effective patient care post-discharge. An exciting opportunity has arisen for an experienced Registered Nurse to join our dedicated Discharge Team at Wexham Park Hospital. This role offers the chance to further develop your skills in assessment, care planning and multi-agency working, while making a real difference to patients and their families.
As a Discharge Facilitator, you will play a key role in supporting the safe and effective discharge of patients, with a particular focus on those with complex health and social care needs or where discharge may be delayed. You will contribute to the management of Discharge to Assess pathways, ensuring patients are supported in the right place, at the right time, with the right level of care.
Key responsibilities include:
Assisting with the co-ordination and planning of complex discharges across all clinical specialties.
Supporting wards and clinical teams in identifying barriers to discharge and helping to resolve them.
Working closely with patients, families, and partner agencies to ensure safe, person-centred discharge planning.
Supporting timely patient flow through the hospital by promoting effective discharge processes.
What we're looking for:
A Registered Nurse with proven post-registration experience.
Strong communication and negotiation skills, with a flair for problem-solving.
The ability to remain calm and effective under pressure in a fast-paced environment.
A flexible, proactive approach, with the confidence to work both independently and as part of a multidisciplinary team.
A commitment to high standards of patient care and collaborative working.
