Job description

An exciting opportunity has arisen for an experienced Registered Nurse to join our dedicated Discharge Team at Wexham Park Hospital. This role offers the chance to further develop your skills in assessment, care planning and multi-agency working, while making a real difference to patients and their families.

As a Discharge Facilitator, you will play a key role in supporting the safe and effective discharge of patients, with a particular focus on those with complex health and social care needs or where discharge may be delayed. You will contribute to the management of Discharge to Assess pathways, ensuring patients are supported in the right place, at the right time, with the right level of care.

Main duties of the job

Key responsibilities include:

Assisting with the co-ordination and planning of complex discharges across all clinical specialties.

Supporting wards and clinical teams in identifying barriers to discharge and helping to resolve them.

Working closely with patients, families, and partner agencies to ensure safe, person-centred discharge planning.

Supporting timely patient flow through the hospital by promoting effective discharge processes.

A Registered Nurse with proven post-registration experience.

Job responsibilities
  • To promote the continuity of care of patients with complex needs discharged from hospital to community
  • To provide information, advice and support to health care professionals, patients, relatives and others involved in patient care
  • To support the MDT to ensure timely and appropriate decisions in the discharge process
  • To facilitate the planning of complex discharges where sensitivity and persuasive skills are needed
  • To develop working relationships with Social Services and community staff to enable discharge plans to be put into practice
  • To gain the knowledge to identify delays in the patient pathway and resolve problems identified
  • To undertake Continuing Health care assessments, including both decision support tools and checking checklist prior to submission with other professionals
  • To provide legible and comprehensive patient records to agreed Trust and NMC guidelines
  • To facilitate a patient focused approach to care that promotes respect and dignity
  • To ensure that multi-disciplinary staff practice direct patient care in accordance with professional codes high standards of patient care.
  • Provide support and advice to relatives and carers and ensure effective channels of communication are evident between clinical staff, patients, relatives and carers, being sensitive to the needs of those with sensory impairments or learning disabilities.

  • Broad clinical experience relevant to the post and experience in older people's health
