A significant healthcare organization in Scotland is seeking a Diabetes Specialist Nurse or Practice Nurse to deliver Type 2 Diabetes care and education. The role involves close collaboration with various healthcare professionals, developing educational programs for newly diagnosed patients, and promoting effective diabetes self-management. Candidates must be registered nurses with relevant professional development and possess strong communication and organizational skills. This is a fixed-term position requiring 18.5 hours per week.

Work Pattern – Mon & Tues (9 am–5 pm) and one other half day to be negotiated
An exciting opportunity is available for a Practice Nurse with a proven interest in Diabetes or a Diabetes Specialist Nurse to be involved in the delivery of Type 2 Diabetes care in NHS GGC.
Based within the Public Health Directorate, the successful post holder will deliver the Type 2 Diabetes education programme Control It Plus – both digitally and in‑person to newly diagnosed patients across NHS GGC, in addition to providing wider support and training to promote improved Diabetes self‑management and care.
This role requires close, autonomous collaboration with other healthcare professionals across both primary and secondary care settings, including Diabetes Specialist Nurses, Dietitians, Podiatrists, and the broader Primary Care support teams.
As part of the role the post holder will increase awareness and engagement in Type 2 diabetes education, care planning, and lifestyle interventions, empowering individuals with diabetes and their carers to manage the condition effectively.
Candidates must be able to travel throughout NHS GGC and occasionally work outside standard hours to meet programme delivery commitments.
