Community Worker
Redeemed Care Inc is a well‑established community service organization providing support services to individuals, families, and vulnerable members of the community across Western Australia. We seek a dedicated Community Worker to support our expanding programs and client base.
Key Responsibilities
- Assess individual and family needs and plan, develop, and implement appropriate educational, training, and support programs tailored to clients’ circumstances.
- Conduct client interviews to assess the nature, extent, and impact of social, emotional, financial, and welfare‑related difficulties.
- Monitor, document, and report on client progress against care plans and support outcomes.
- Refer clients to relevant external agencies and service providers for additional or specialised assistance, including health, housing, employment, and welfare services.
- Assess community needs and available resources relating to health, welfare, housing, employment, training, and social support services.
- Liaise with community groups, welfare organisations, government bodies, and private service providers to address community issues and promote awareness of available resources and services.
- Provide direct support to families and individuals, including children, people with disabilities, and vulnerable adults within community and supported service environments.
- Assist young people to address and resolve social, emotional, and financial challenges through structured support and referral pathways.
- Prepare written reports, case notes, and submissions relating to client services, funding requirements, and program outcomes for internal use and external stakeholders.
Essential Requirements
- Relevant qualification: AQF Associate Degree, Advanced Diploma or Diploma in Community Services, Social Work, or a related field (or equivalent experience).
- 1–3 years of relevant experience.
- Demonstrated experience working with diverse and vulnerable client groups.
- Strong interpersonal, communication, and case‑management skills.
- Ability to work collaboratively with internal teams and external stakeholders.
- Sound understanding of community services frameworks and referral networks.
