A community health organization in Oakengates, UK, is seeking a Registered Nurse to provide planned care in the community. The ideal candidate will possess community nursing experience and evidence-based practice knowledge, emphasizing teamwork and professional development. Responsibilities include assessing patient needs, providing clinical support, and educating patients. This role covers a 7-day service on a rota basis, with a commitment to delivering high standards of care.
An exciting opportunity has arisen for a highly motivated Registered Nurse to join the North Telford District Nursing Team, in the provision of Planned and Admission Avoidance care to patients in the community.
We are looking for a caring, motivated friendly community nurse to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.
Ability to travel independently to fulfil the requirements of the post is essential.
The base for this position is Telford, however, Community working will be based in Newport, Shropshire.

The post is covering 7 days 8am to 6pm service, on a Rota System.
