Overview

We are seeking to recruit a newly qualified or experienced Band 5 community nurse to join our team. We want you to be a person who values development, be self led and confident. Responsibilities
  • Deliver high-quality nursing services and holistic assessments of patients with a person-centred approach.
  • Work alongside various professionals to provide quality and seamless care to patients predominantly in their own home.
  • Proactively coordinate care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
Organisation and location

Care community team is based in Ellesmere Port and covers the local area. Access to transport for work purposes is important; mileage expenses are reimbursed in addition to hourly rate. Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, with services across Wirral, Cheshire, Trafford, Warrington, Bolton, Halton and Liverpool.

Requirements / Qualifications
  • Band 5 community nursing registration; newly qualified or experienced welcomed.
  • Ability to work in a fast-paced, patient-centred environment.
  • Access to own transport.
  • Willingness to engage with CWP values and a values-based recruitment process.
  • DBS disclosure as applicable; applicant pays for DBS check with options for DBS Update Service.
  • Evidence of appropriate behaviours as per the person specification and Trust values.
Induction and recruitment

All new starters commence on a fixed start date each month in line with Prepare to Care (P2C) induction, offering up to 3 weeks training to meet induction requirements.
