A healthcare organization in the UK seeks an experienced nurse for a leadership role in a community setting. The successful candidate will assess care needs, delegate care, supervise staff, and ensure high standards for patients. This position requires excellent clinical skills and the ability to travel between sites. Full-time applications are preferred, though part-time options will be considered. An interview date is set for January 21, 2026.
Post holder will assess care needs and develop, implement and evaluate programmes of care independently. The role involves ensuring high standards of care are delivered to patients and their families/carers within the home or clinic environment.
Delegate the delivery of care where appropriate, in line with the NHS Wales delegation framework. Supervise the delivery of care and staff involved with its provision. Ensure that the patient’s physical, psychological, social, spiritual and fundamental aspects of care needs are met.
Be accessible in the community setting to the nursing and wider multi‑disciplinary team, patients and service users within primary and secondary care. Continually support and work in conjunction with the Senior Sister / Charge Nurse and formally deputise in their absence. Deal with complex clinical and managerial situations that arise, critically analysing, seeking appropriate help and making decisions in relation to patient risk and care.
Seek appropriate advice and alert agencies when there are any concerns regarding potential or actual risk to any patient or their dependents (e.g., child safeguarding, domestic violence, protection of the vulnerable adult, mental capacity and deprivation of liberty).
An exciting opportunity has arisen for an experienced nurse with leadership experience to further develop these skills within a Community Setting, without the requirement of the Specialist Practitioner Qualification. The role requires the successful applicant to combine excellent clinical practice with day‑to‑day shift coordination, in addition to undertaking leadership and management responsibilities in support of the District Nurse Team Leader. Formal deputisation in the absence of the District Nurse Team Leader is required.
Due to the rural nature of our communities the post holder will need the ability to travel between sites in a timely manner. The position is full‑time; part‑time will be considered.
