Overview

The CRHT is a multidisciplinary (MDT) team that provides 24/7 urgent clinical triage and advice via the crisis telephone line, as well as conducting urgent and emergency mental health assessments for people in crisis. The team also provides intensive home treatment to patients on the caseload as an alternative to hospital admission, and acts as gatekeepers for inpatient admissions, crisis house admissions, and facilitates early discharge from inpatient wards. It supports a safe environment that maintains the physical and mental well-being of patients and staff.

To assist in maintaining a culture of respect, privacy and dignity for all, and to deliver care that is safe, effective and evidence-based without direct supervision. The team actively supports joint Health and Social Care interventions, ensuring cooperation and collaboration at all times.

Responsibilities
  • Lead a team of Support Treatment and Recovery Workers in delivering continuous, effective evidence-based care.
  • Carry out comprehensive assessments including risk assessment on admission to the team and at appropriate intervals thereafter.
  • Devise and implement individual care strategies in conjunction with the client, caregiver and MDT, regularly evaluating and modifying the strategy as required.
  • Provide a high standard of care in accordance with Trust policies and procedures.
  • Understand the principles of the nursing process and care programme approach and complete documentation to the required standard.
  • Carry out clinical procedures related to patient care and contribute to the training of junior staff in these procedures as appropriate.
  • Ensure the correct administration of drugs in line with Trust policy and legal requirements.
  • Actively participate in the Trust observation/engagement policy and be aware of the patient’s whereabouts as required.
