A healthcare organization in Scunthorpe is seeking a Team Leader for their mental health crisis team. The role involves leading support workers in delivering effective, evidence-based care, conducting assessments, and ensuring the participants' well-being. Ideal candidates will have experience in mental health settings and a thorough understanding of the nursing process. This position offers the chance to make a meaningful impact in the community.
The CRHT is a multidisciplinary (MDT) team that provides 24/7 urgent clinical triage and advice via the crisis telephone line, as well as conducting urgent and emergency mental health assessments for people in crisis. The team also provides intensive home treatment to patients on the caseload as an alternative to hospital admission, and acts as gatekeepers for inpatient admissions, crisis house admissions, and facilitates early discharge from inpatient wards. It supports a safe environment that maintains the physical and mental well-being of patients and staff.
To assist in maintaining a culture of respect, privacy and dignity for all, and to deliver care that is safe, effective and evidence-based without direct supervision. The team actively supports joint Health and Social Care interventions, ensuring cooperation and collaboration at all times.
