A community health organization in Plymouth is seeking a qualified Mental Health Practitioner to join its Functional Older People's Mental Health team. Livewell Southwest is seeking an enthusiastic professional to join ourFunctional Older Peoples Mental Health (OPMH) community team. This is afantastic chance to work with older adults experiencing functional mentalhealth conditions such as depression, anxiety, schizophrenia, and bipolardisorder, and to make a real difference to their lives.
The OPMH Functional Team is a city-wide service providingspecialist mental health assessment and evidence-based interventions for peopleaged 65 and over. This role is ideal for a qualified mental health practitionerwith an interest in older peoples care, but we also welcome applications fromnewly qualified professionals or those due to qualify soon.
To succeed, you will need excellent communication skills,empathy, and the ability to work effectively within a multidisciplinary team.You will have access to supervision and support from experienced colleagues andopportunities for professional development within a forward-thinkingorganisation.
In this role, you will act as a keyworker for a definedcaseload under the Personalised Care Framework, carrying out initialassessments, developing care plans, and delivering interventions that promoterecovery and wellbeing. You will respond to duty calls, liaise withpsychiatrists for prescription requests, and work collaboratively with othermental health teams, inpatient services, adult social care, GPs, third-sectororganisations, and families to ensure holistic, person-centred care.
Thepost holder may be required to supervise junior staff within the team andassess/mentor students.

KeyTasks and Responsibilities of the Post:
The post holder will assess all new referrals asappropriate, at home or in a clinic setting, in accordance with the agreedOperational Policy.
The post holder will provide nursing care and otherinterventions as appropriate in the right setting, in accordance with theagreed Operational Policy.
The post holder will arrange for a defined group ofpractice patients to access other Mental Health Services and work closely withall other clinical professionals e.g. Consultant Psychiatrist, Clinical Psychologist,Social Worker etc.
The post holder will act as the keyworker for anallocated group of patients and organise care in accordance with the PersonalisedCare Framework.
The post-holder will take part in the clinical audit ofthe team. This will include annual Peer Reviews and Infection Control audits.
The post holder will ensure that their clinical practiceis based on evidence and good practice and in accordance with any NICEguidance.
The post holder will be individually accountable fortheir practice in accordance with the NMC or HPC professional code of conduct.
StaffManagement:
The post holder may be responsible for supervising somejunior staff within the team.
Liaisonand Administration:
The post holder should maintain close links within-patient services at Glenbourne, Home Treatment Team, First Response Service,Social Care, Plymouth Dementia Pathway, Dual Diagnosis Services, Primary Care,Counselling Services, Liaison Services at Derriford and Mount Gould hospital.Other services might also be applicable from time to time e.g. the Forensic andAsylum Seekers teams.
The post holder will keep up to date electronic/paperrecords of all patient and staff activity, ensuring these are kept in a safeplace, in accordance with NMC or HPC guidelines, and Livewell Southwestpolicies including Data Protection Act and the Mental Health & MentalCapacity Acts. The post holder will be aware of NMC/HPC and Livewell Southwestguidelines on record keeping, confidentiality, Mental Health Act and DataProtection Act.
Professional Development and Education:
The post holder will be required to keep up-to-date. Thiswill include mandatory training and developments in mental health nursing orsphere of practice.
The post holder will participate in the provision ofrelevant community placements and training programmes for nurses.
The post holder will be required to ensure that theyreceive appropriate regular line management and practice supervision in linewith current policies of Livewell Southwest.
The post holder will participate in an annual appraisal;this Job Description will be reviewed at this time.
