Job description
Community Mental Health Nurse

The closing date is 29 December 2025

To provide high quality mental health care, you will assess, support and monitor service users who experience mental health issues. You will work in a variety of community settings, liaise and co‑work with statutory and non‑statutory services, and engage with carers, families and service users in their homes, clinics, hostels, supported living and residential settings.

Main duties of the job
  • Lead professional role: plan, deliver, coordinate and evaluate assessments and formulation.
  • Collaborate closely with team members including therapists, medical professionals, nurses, allied health professionals and support staff.
  • Conduct complex person‑centred assessments.
  • Ensure evidence‑based care, addressing physical, cultural, psychological and spiritual needs of users.
  • Assess care needs, devise care packages including risk assessment, PROMs and implement and evaluate these in collaboration with users and carers.
  • Support and mentor less experienced staff, support workers and students on placement.
  • Undertake duties extending clinical and managerial expertise as delegated.
  • Undertake comprehensive outcome monitoring of all referrals.

Experience
  • Appropriate experience either pre or post qualification in mental health.
  • Crisis working and management of complex needs.
  • Experience in a multi‑disciplinary setting.
  • Local government experience, mental health experience, Care Programme Approach and care coordination.
  • Experience collaborating with older people with mental health problems.
Qualifications
  • Degree, Diploma or equivalent in mental health and significant pre‑qualification experience plus additional training or short courses.
  • Continuing post qualification mental health training.
  • Experience working with people with mental health problems.
Knowledge
  • Understanding of Equal Opportunities policy.
  • Knowledge of the National Health Service and related organisations.
  • Knowledge and understanding of current mental health issues.
  • Knowledge of the Mental Health Act, Mental Capacity Act and Care Act including other relevant legislation.
  • Understanding of the Care Programme.
  • Approach and ability to fulfil a care coordinator role.
  • Assessment and organisational skills.
  • Ability to prioritise work.
  • Range of intervention skills.
  • Evidence of coordinating/planning interagency approaches to mental health care.
