The Successful candidatewill be anenthusiastic,ambitiousandexperienced Registered Nurse,whois keento advance their clinical, professional and leadership skills.You will be expected todeliver excellenthighly specialistpatient centredleg ulcer treatmenttopatientswho are able to attend clinics across the Dudley Borough.
To be successfulyouwillneed to have experience and knowledgeinLeg Ulcer treatment and anunderstanding of alltheNational and Local guidelines and pathwaysrelevant tothis cohort ofpatients.
You will haveresponsibility fordeveloping relationships with allstakeholdersto ensure a seamlessapproach to the care ofpatients with leg ulcers, working closely with district nurses and primary care services.
You willassist ineducatingcolleaguesintheimplementation of integrated pathways of care and delivering on training packages and supervision/mentoring.
You will beorganized, confident, ableto work independently, and be anexcellent communicator.
The post-holderwill carry outassessmentof patients,planningand implementationoftreatment Pathways, within the service,supporting the team withongoing assessment of care needs and the development,implementationand evaluation of programmes of care. The post holder will carry out all relevant forms of care and will take responsibility for caseloadon behalf oftheTeamLeader intheir absence.
Accurately andtimelyrecording of all care given to the patients and report any changes in the patient's condition to the general practitioner or other members of the health / social care team and that may be used in enquiries.
Attending andparticipatein staff meetings, case conferences and other meetings asrequired.
Ensuring that quality standards of care aremaintainedusing audit and other monitoring systems.
Contributing to the local Clinical Governance agenda being aware ofDudleyCommunity Services and local priorities / requirements.

Having responsibility for the health assessment of theall-agegroups within the population as their needs arise.
Using the knowledge and skills necessary to assess individuals and groups,identifyingthe multiple needs of the patient, family / carer giving inaholistic way taking into consideration cultural differences.
Working within the primary care setting as a member of the health care team,participatingin activities, which address the health needs of the general practice caseload.
Providing andmaintaininga high standard of skilled nursing care forpatients'health centres and practice surgeries using a nursing model of care, that is consistent with best evidence, within own scope of practice and legislation.
Teaching nursing procedures to relatives / carers so that the care of the patients may be continuous over 24hours, andgive guidance in carrying out all treatments.
Ensuring the changing needs of individuals and groups areidentifiedtimelyand adjustments to programmes of care are made.
Promoting andmaintainingoptimumhealth byidentifying,planningand undertaking specific health promotional activities with identified individuals and target groups.
Taking part in preparing an annual caseload profilein order toidentifythe health needs of the area and using the information to inform other professionals and to direct development of services.
Following Dudley Community Services guidelines in all suspected and confirmed emotional,sexualand physical abuse.
Ensuring that concerns andidentifiedpotential risks are referred to theappropriate MedicalPractitioner / Agencyimmediately.
Complying withand helping to fulfil theobjectivesof the primary health care team.
Participating in the setting of personalobjectiveswith the Caseload holder through the job appraisal and clinical supervision processes.
Maintaining and continually update professional knowledge and skills
Participating in the training of District Nursing and nursing students and others as directed, supporting them to apply theory into practice.
Demonstrating nursing procedures where applicable.
Assistingin the induction programme fornew staffto Dudley Community Services.
Ensuring fulfilment of the competencies relating to the Nurse prescribing competency frameworkaremet (If applicable).
Reviewing information on services andidentifyingareas for development.
Co-operating andassistingin research and survey programmes relating to the client group or the nursing profession.
Ensuring that quality standards of care aremaintainedusing audit and other monitoring systems.
Contributing to the local Clinical Governance agenda being aware ofDudleyCommunity Services and local priorities / requirements.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
