Join Our Caring Community First Team – Community Health and Social Care Support Worker Opportunities

Are you passionate about making a real difference in people’s lives? We’re looking for compassionate and dedicated Community Health and Social Care Support Workers to join our Community First Team at the Perth House Assessment Hub in Derby.

At Perth House, we provide vital short‑term support to individuals who have recently been discharged from hospital or require urgent care in the community. Our goal is to help people maximise their independence in a safe and supportive environment — and you can be part of that journey.

Working Hours
  • Morning shift: 7:15 AM – 2:45 PM
  • Evening shift: 2:30 PM – 10:00 PM

Shifts include weekends and bank holidays.

Base

Perth House Assessment Hub, Athlone Close, Chaddesden, Derby, DE21 4BP.

Responsibilities

The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.

  • Maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.
  • Assist all clinicians in developing agreed programmes of treatment and care where appropriate and to work as an effective member of the wider multi-disciplinary team.
  • Carry out individual care without direct supervision in the Perth House Assessment Hub.
  • Provide support to patients to optimise their health and independence.
The purpose and activities of DCHS

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes, and increasingly, in people's own homes and via virtual consultations.
