You will bepart of a dynamic team, committed to develop the role of a clinical pharmacistin an expanding field of General Practice to its maximum potential to deliverperson-centred, coordinated care across the health system.
This post wouldrequire both, to work as part of a multi-disciplinary team in a patient-facingrole, but also individual clinical decision making around your caseload ofpatients.
Theobjective of the Pharmacist is to contribute to highquality, personcentred care across South HardwickPrimary Care Network by delivering expert clinical pharmacy services withinGeneral Practice.
Thepost holder will work within their clinical competencies, to strengthenmultidisciplinary working through patientfacingclinical decisionmaking,comprehensive medication reviews, and support for chronic disease management,polypharmacy, and PCN contractual priorities.
Thepost holder will enhance medicines optimisation, respond to prescription andmedication queries, and provide clinical leadership to improve systems,quality, and safety across practices.
South Hardwick PCN is a group of 8 forward thinking and progressive practices (population 69,372) who have developed a friendly and effective working relationship with one another. We are committed to working collaboratively to improve outcomes for patients and sustaining General Practice.
South Hardwick is a mix of former mining communities, suburbs, and rural communities to the south of Chesterfield. Your employer would be North Eastern Derbyshire Healthcare Ltd, and you would be entitled to NHS pension.
We have a well-established Pharmacy team of six Clinical Pharmacists and seven Pharmacy Technicians.
Reviewpatients with single or multiple medical problems integrating into existingGeneral practices teams long term conditions clinics (e.g. Hypertension,Diabetes, Heart disease, Asthma.).
Reviewmedications for these patients assessing on-going need for each medicine, areview of monitoring needs and to support patients ensuring they get the bestuse of their medicines (i.e. medicines optimisation)
Undertakeclinical medication reviews of specific groups of patients identified aspriority, such as, medications more likely to cause harm, severe frailty,potentially addictive medications, Atrial Fibrillation anticoagulationreviews, gastroprotection reviews and others identified by the practice aspriority.
Domiciliarymedication reviews(houseboundor care home residents)
Undertakeclinical medication reviews with these patients, liaison with the Communitysupport teams if necessary
Patientfacing /telephone medicines support
Providepatients advise with questions, queries, and concerns about their medicinesin the practice.
Medicineinformation to practice staff and patients
Answersrelevant medicine-relatedenquiries from GPs, other practice staff, other healthcare teams (e.g.community pharmacy). Suggesting and recommending solutions. Providing followup for patients to monitor the effect of any changes
Un-plannedhospital admissions
Review theuse of medicines most commonly associated with unplanned hospital admissionsand readmissions through audit and individual patient reviews. Put in placechanges to reduce the prescribing of these medicines to high risk patient groups.
Signposting
Ensure thatpatients are referred to the appropriate healthcare professional for theappropriate level of care within an appropriate period of time e.g. pathologyresults, common/minor ailments, acute conditions, long term condition reviewsetc
Clinicalsupervision and support
To Pharmacytechnicians and Foundation pharmacists within the team
Repeatprescribing
Support therepeat prescriptions system in practices, reviewing medicines reaching reviewdates and flagging up those needing a blood test or a review. Ensure patientshave appropriate monitoring tests in place when required.
Riskstratification
Identificationof practice and PCN priorities through data analysing and prioritise patientsto review.
To usemedicines data to highlight issues and risks to support decision-making
Servicedevelopment
Contributewith pharmaceutical advice for the development and implementation of newservices that have medicinal components (e.g. advice on treatment pathwaysand patient information leaflets).
MedicinesQuality Improvement
Undertakeclinical audits of prescribing in areas directed by the GPs, feedback theresults and implement changes in conjunction with the practice team
Support andhelp the technical team implement changes to medicines that result from MHRAalerts, product withdrawal and other local and national guidance.
Educationand Training
Provideeducation and training to peers and other primary healthcare team ontherapeutics and medicines optimisation.
CareQuality Commission
Work withthe general practice team to ensure the practice is compliant with CQCstandards where medicines are involved.
PublicHealth
To supportpublic health campaigns.
To provide specialist knowledge on all public healthprogrammes available to the general public.
