Job summary

A primary care network in Sheffield is looking for a qualified clinical pharmacist. The position involves managing prescriptions, providing clinical advice, and collaborating with general practice teams to deliver high-quality patient care. Ideal candidates will have an undergraduate degree in pharmacy, registration with the General Pharmaceutical Council, and at least 2 years of pharmacy experience. Qualifications

• Minimum of 2 years experience as a pharmacist.
• Able to work flexibly across practices with a multidisciplinary team.
• Understanding of the pharmacy role in governance.

Responsibilities

• Manage prescription and medication-related queries.
• Contribute to the repeat prescription process.
• Carry out medicines reconciliation during transfers of care.
• Provide clinical medication reviews.
• Identify cohorts of patients at risk of harm from medicines.
