Role Overview

We have a vacancy for a Clinical Pharmacist to support a number of services being delivered across our Frailty Service based at the Jean Bishop Integrated Care Centre in Hull.

Key Responsibilities

Clinical Pharmacists work with a team of doctors, nurses and therapists providing a comprehensive geriatric assessment to patients seen in the following settings:

  • Jean Bishop Integrated Care Centre
  • Care homes
  • Community bedded units
  • Frailty hospital at home (virtual ward)

We can offer part‑time hours (2 or 3 days) and flexible working to support our current services and projects within the Frailty team.

Qualifications & Experience

Experience of performing structured medication reviews is preferable but not essential. Successful candidates will demonstrate:

  • Patient‑centred care approach
  • Interest in frailty
  • Excellent communication and organisational skills

Applicants are welcome from all pharmacy sectors. You will receive professional support from our established Medicines Service team based in Hull.

Additional Requirements

You must have an independent vehicle to travel across the geographic area of our services and be able to work flexibly to meet organisational needs.
